Advanced Care Planning
What is an Advanced Care Directive?
An Advanced Care Directive puts your wishes about care, finances, and final arrangements in writing while you're still able to decide. Sharon helps you prepare clear, written documents covering:
Requests — your specific wishes and preferences
Legal — powers of attorney and decision-making authority
Medical — your preferences for treatment and terminal care
Respect — how you want to be treated and remembered
Financial — guidance for managing your affairs
Comfort — what you need to feel safe and at peace
Spiritual — your faith, values, and beliefs
Funeral plans — your wishes for end-of-life arrangements
When your wishes are documented ahead of time, the people who love you don't have to guess.
